Nurse - PACT - PACT (PACT, Boston)
Schedule: Mon - Fri (potential for some rotating weekends)
Salary: $70K
Responsibilities:
PACT operates with the Assertive Community Treatment model and is a multidisciplinary team comprised of a Nurse Practitioner, nurses, clinicians and specialists able to support people in mental health recovery in finding housing, managing medical issues, dealing with addictions, integration in the community and rejoining the workforce or educational programs.
Clients are served in their homes and other community settings.
The nurse is responsible for maintaining a caseload as well as supporting clients with their medication, physical health and coordination of their medical needs.
Qualifications:
RN; Master's, Bachelor's or Associates in nursing; previous experience working with individuals with severe and persistent illness; Acceptable CORI, valid driver's license and available car
