Job Description:

To manage and coordinate the care of assigned clients in their homes by providing comprehensive skilled nursing services as needed to achieve maximum rehabilitation and recovery for the client

Responsibilities Include:
• Completes Initial comprehensive assessment of clients’ needs in order to achieve determined goals
• Accept client admissions as assigned by agency
• Provide all required skilled services as ordered by the physician
• Observe signs and symptoms and report to the manager as needed to achieve patient goals
• Oversight of collective care team to achieve common goals for the patient
• Promotes client and caregiver independence in self care
• Maintains comprehensive administrative, and medication records
• Consults with physician when necessary to achieve patient goals

Skills

Requirements  
Education and Experience:
• Holds current license for state location of employment
• Minimum of 1 year of experience in Home Health Setting preferred
• 2 years of experience in acute care setting that requires comprehensive assessment skills
• Excellent communication skills
• Experience in care coordination between, physician, clients, referral sources
