Case Manager Skills and Qualifications
-Ability to develop and deliver presentations.
-Skill in the analysis and re-engineering of systems, processes, and procedures.
-Knowledge of nursing and case management theory and practice.
-Ability to make administrative decisions and judgments.
-Knowledge of patient care charts and patient histories.
-Knowledge of clinical and social services documentation procedures.
-Ability to supervise and train employees.
-Organizing and coordinating skills.
-Ability to communicate technical information to non-technical personnel.
-Knowledge of billing procedures for clinical and social services.
-Ability to work independently, as a team member, and/or as a team leader.
-Ability to develop, implement, and modify health care plans.
Case Manager Job Responsibilities:
Bringing client/family access to health care, school, and community resources; assists client/family to develop natural resources and make contact with social support network. Develops resources as appropriate to meet client/family needs in their community. Works independently with clients and/or families to assess, develop, implement, monitor, and recommend modifications to comprehensive, cost-effective care plans, using a multidisciplinary process. Coordinates, monitors, and evaluates provider services and care, to include home and/or school visits, physician/hospital visits, and therapy visits, for a specified client population.
