Board Certified or Board Eligible - REQUIRED
Active MD State License - REQUIRED at time of submission, no exceptions
Active MD DEA and CDS - REQUIRED at time of submission, no exceptions

Dates of Coverage: 8/11/25 (or as soon as credentialed) - ongoing until Perm is hired
Schedule (2 options): M-W-F 8 hr/day coverage + 7 24 hours on-call shifts per month OR M-F 8 hr/day + 7 24 hours on-call shift per month.
Call Schedule: 1 week of call per month (7 consecutive 24 hour shifts)

Required Procedures:
Chemotherapy - Intracavitary - Intrapleural / Peritoneal, etc.
Chemotherapy - Intrathecal
FNA/Biopsy
Mgmt. of VAD/CV Catheters
