Responsibilities:- Provide skilled nursing care in patients’ homes
- Monitor vital signs and patient condition
- Administer medications and treatments as ordered
- Perform wound care and basic procedures within scope
- Educate patients and caregivers on care plans
- Document visits and patient progress accurately
- Report changes in condition to RN or provider
- Follow infection control and safety guidelines
