Requirements:
• Active MI License or willing to obtain
• BC in Hematology & Medical Oncology
• DEA
• ACLS, PALS
• Clean Record
• Most recent NPDB Report
• Must be experienced physicians
• Completion of Hematology/Oncology Fellowship
