Description
A social services organization in New York City is looking to add a new Case Manager to their team in Brooklyn. In this role, the Case Manager will be responsible for facilitating engagement with shelters to reduce adverse substance use incidents in shelters (accidents, altercations, emergency services and overdoses) in order to improve client health and housing outcomes by providing and connecting at risk individuals to clinical services.
About the Opportunity:
- Start Date: ASAP
- Assignment Length: Ongoing (Contract to Hire)
- Schedule: Monday to Friday
- Hours: 9pm to 5pm (one 7pm evening per week)
Responsibilities
The Case Manager will:
- Provide case management on site to people living in the shelter that are at risk of harm from drugs and alcohol
- Provide recovery support services to individuals identified as at risk
- Provide coordination of treatment and transitional planning, as needed
- Provide harm reduction education and awareness to shelter staff and clients on a regular basis: presentations, office hours, signage, etc.
- Assist with making referrals to harm reduction, substance use, primary care, mental health care, pain management or other, as needed.
- Support linkage to care, return to care, retention in care
- Support discharge and transition planning
- Keep accurate documentation and reporting of services delivered (in CARES).
- Perform other duties, as needed
Qualifications:
- 2+ years of experience with individuals with Mental Illness, Substance Abuse issues, and/or history of Homelessness
- Bachelor's Degree in Human Services or related field
- Computer savvy
- Microsoft Office proficient (Excel, PowerPoint, Word, etc.)
Desired Skills:
- Master’s Degree in Human Services or related field
- Bilingual (English and Spanish)
