Job Description:

• Schedule: 5 days per week, 40 hours per week, Monday to Friday, 7:00 am to 3:00 pm, plus lunch break, no weekends


• Setting: Multi-specialty surgery center


• OR Facilities: 7 OR’s, 2 procedure rooms


• Supervision: MD’s on-site supervise CRNA’s


• Cases: General, Ortho, Plastics, ENT, GI, GYN, Eyes, Pediatrics


• Provider Ratio: 2-3 docs on-site; CRNA's 8-10 daily


• Charting: Paper charting, no EMR


• Daily Cases: Range anywhere from 45-80 per day


• Duration: 3 months

Requirements:

• Board Certification: NBCRNA, ACLS required


• License Requirement: Must have Active FL license


• Privileges: 60 days, Clean Files Only


• Anesthesia Modality Checklist: Completed by the provider within 6 months


• CV: Must be in complete month/year format covering all gaps of 30 days or more


• NPDB Self-Query: Required for all providers who have malpractice claims or background issues


• Completed EVPS Disclosure Form


• Clinical Interview: May be required

State License Requirements:

• Florida
