• Includes some Chronic Pain Management


• Chart closure within 72 hours


• Schedule = 18 PPD, minimum to see 18 PPD


• Monday-Friday 8-5


• Mix of peds/adults


• A decision is typically made within 7-14 days


• APC's with 3 years of experience in a similar setting


• PAs must have CA Controlled Substance Education Course Certificate


• NPs must have Furnishing License with Schedules II


• Active DEA needed before start


• Credentialing takes 30-60 days (six weeks is standard for Locums who submit app./documents timely)


• The following immunizations are required for credentialing (before the start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before the start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years)
