• Schedule = 18 PPD, minimum see 18 PPD


• Monday-Friday 8-5


• A mix of peds/adults (Will consider IM only)


• A decision is typically made within 7-14 days

• BE will be considered if truly BE (5 years or less since residency)


• Active DEA needed before the start


• Credentialing takes 30-90 days (six weeks is common if the provider submit app./supporting documents timely)


• The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).
