• Required immunizations for credentialing: 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before the start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years), COVID19 vaccination, and booster (no longer required).
