Physical Therapist - Home Health - (PT - HH)

Travel PT Home Health jobs in Salem, OR are available for immediate start with 13-week assignments, working 8-hour days and seeing 5 to 6 visits per day. You must have an active Oregon license and home health experience.
Your responsibilities include assessing patient needs, developing individualized treatment plans, providing one-on-one therapy in patients’ homes, and documenting progress in Homecare Homebase EMR. You will help patients improve mobility, reduce pain, and enhance independence, while coordinating care with other healthcare professionals
