Our client in California is seeking a Family Practice Physician Assistant for a 135-day assignment starting Jun 22, 2026. This role involves working 10am - 7pm shifts, focusing on assessing and managing acute and chronic conditions, comprehensive patient screening, health education, and coordinating patient care.

Responsibilities and Duties

• Assess and manage acute and chronic conditions and participate in disease registry.
• Screen all patients for health maintenance needs, including but not limited to immunizations, cancer screening, and behavioral health screening.
• Educate patients in health promotion, prevention, and birth control as appropriate.
• Provide preliminary telephone consultation to patients, when requested.
• Contact patients for follow-up care in case of life-threatening illness.
• Help coordinate efficient flow of patients through the system of care.
• Actively monitor and appropriately direct referrals placed for patient care.
• Comfortable working with LGBT, unhoused, and low-income patient populations.
• Approximately 40% of cases will involve STI/HIV screening.
• Obtain and review patient histories, develop patient care plans, and ensure completeness and accuracy of health records.
• Order and interpret in a timely fashion results of laboratory, radiology, and other tests.
• Proactively assist patients through all facets of accessing care.
• Provide Medication-Assisted Treatment (MAT).
