This nonprofit operates three clinics between Lewiston and Augusta, each about one hour from Portland. Care is delivered in a nationally recognized Patient-Centered Medical Home model, with integrated behavioral health, care managers, and patient assistance coordinators. This is a well-established, modern practice supported by experienced physicians and a collaborative care team.

This full-time position (4 days/week, 40 hours) offers the opportunity to assume an established patient panel with a high level of acuity, providing comprehensive outpatient primary care to patients of all ages, with more seniors than pediatrics. You will support meaningful, long-term multigenerational patient relationships in this well-established practice.
