Monmouth, Maine: A respected Federally Qualified Health Center serving a multicultural rural population of more than 10,000 people in central Maine seeks an experienced Family Nurse Practitioner or Physician Assistant to join its established primary care team.This nonprofit operates three clinics between Lewiston and Augusta, each about one hour from Portland. Care is delivered in a nationally recognized Patient-Centered Medical Home model, with integrated behavioral health, care managers, and patient assistance coordinators. This is a well-established, modern practice supported by experienced physicians and a collaborative care team. This full-time position (4 days/week, 40 hours) offers the opportunity to assume an established patient panel with a high level of acuity, providing comprehensive outpatient primary care to patients of all ages, with more seniors than pediatrics. You will support meaningful, long-term multigenerational patient relationships in this well-established practice.
