• Board: Board Certified
  
• Licenses: Licensed in WI or Universal License (IMLC)

Duties & Responsibilities

• Provide outpatient pain medicine services including office visits, follow-up visits, and procedures
  
• Perform spinal cord stimulation, radio-frequency ablation, and injections
  
• Work a flexible schedule of 2 weeks on/2 weeks off, 5 days per week, 7a-3p
  
• Collaborate with support staff (RNs and office staff)
  
• Maintain accurate documentation in Epic EMR
