Requirements:
- 2+ years of experience
- Current unrestricted Registered Nurse license in Colorado
- Basic Life Support (BLS) certification
Duties & Responsibilities:
- Help patients identify their personal goals of care
- Educate and build confidence in caregivers caring for family members
- Serve as a generalist on all diseases and specialist in symptom management
- Excel in clinical assessment and critical thinking
- Manage an assigned caseload of patients and lead the interdisciplinary team
- Provide care wherever the patient calls home, including private residences, nursing homes, group homes, and assisted living facilities
