What You’ll Do
- Screen, evaluate, and provide speech and language therapy for communication, cognitive, and swallowing disorders.
- Develop and implement individualized plans of care aligned with regulatory and clinical practice standards.
- Use standardized assessments and evidence-based practice to guide clinical interventions.
- Monitor progress, adjust treatments accordingly, and document outcomes in the electronic health record.
- Provide consultation and counseling to patients and their families/caregivers, and collaborate with other service providers.
- Accurately record treatment minutes and complete required clinical documentation per company and regulatory requirements.
- Participate in required meetings and facility/company trainings (e.g., care conferences and utilization/rehab meetings).
- Prepare comprehensive discharge summaries and ensure continuity of care based on evaluation and referral outcomes.
- Support and supervise CFY or student Speech-Language Pathologists as applicable.
What You Need
Required
- Active ASHA certification
- Active State Licensure as a Speech-Language Pathologist
- Strong clinical documentation and communication skills
- Familiarity with standardized assessments and evidence-based therapy practices
Preferred
- Experience in Long Term Care (LTC) settings (not required but helpful)
- Experience supporting or supervising CFY or students (if applicable)
