Key Responsibilities of LMFT

Collaborate with medical staff to coordinate patient care, manage referrals, and support treatment planning and discharge processes.
Assess patients and develop individualized treatment plans in collaboration with the care team.
Provide education to patients, families, and staff, and ensure timely updates to care plans and coordination of care transfers.
Provide psychological assessments, therapy, and resource referrals for patients and families.
Develop and implement appropriate psychological interventions to support patient care.
Launch and help develop the Clinical Social Work Intern Program

Skills

Qualifications for LMFT

Master's degree in Marriage and Family Therapy or closely related field required
Current, unrestricted Licensed Marriage and Family Therapist (LMFT) license issued by the State of California required
Minimum 3–5 years post-licensure clinical experience required
Bilingual in Spanish required
CPR (BLS) certification required, ACLS preferred
Experience working in a Federally Qualified Health Center (FQHC) or community clinic preferred
