JOB SUMMARY:
The RN Care Manager Supervisor assesses appropriateness of intensity of services for patients, making recommendations for change, as needed. Applies utilization review criteria to daily assessments of patients needs. Supports medical staff in securing appropriate level of care for patients. Participates in discharge planning process to ensure continuity of care in the most appropriate setting. Plans, organizes and directs the Case Management Department including but not limited to, policies and procedures in accordance with the established policies of the hospital and in accordance with all applicable laws and regulations, scheduling of staff, preparing and reporting department budget. Coordinates with other departments to provide efficient and appropriate services to patients and hospital staff.

PREREQUISITES
* Graduate of an accredited school of registered nursing with current licensure as a Registered Nurse in the State of Vermont. Bachelor of Science Degree in Nursing required. Master’s Degree preferred.
* Three years experience in a hospital setting required. Utilization management and/or case management experience desired.
* Certification in utilization management expected within 2 years. Excellent verbal and written communication skills required.
