Medix is pleased to partner with a nationally recognized healthcare organization focused on a Value-Based Care model serving senior populations. The goal of this position is for providers to build meaningful, long-term relationships with patients, featuring longer appointment times (45-minute visits) and smaller daily caseloads than traditional fee-for-service groups. Working alongside an attending doctor, you will manage non-complex cases and lead comprehensive health plans. Essential Responsibilities:
• Leading daily 8:00 AM clinical team huddles and evaluating patients starting at 8:30 AM.
• Managing dedicated care coordination aligned with Medicare Advantage guidelines.
• Prescribing medications and documenting care in eClinical Works EMR.
• Allocating the final hour of the day (4:00 PM - 5:00 PM) strictly for administrative catch-up.
