Requirements:
- ABOS Board Certified or TRULY Board Eligible in Orthopedic Spine Surgery (must become certified within 5 years) – REQUIRED * no exceptions
- Valid IL & IA license or Active IMLC Willing to Obtain Licenses - REQUIRED
- Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the field of practice- REQUIRED
- DEA - REQUIRED
- BLS Certification and maintain recertification - REQUIRED
- Clean malpractice and license history – HIGHLY PREFERRED / PREFERRED
- Does the provider require visa support – No J-1 or H-1B (Note in Additional Submission Details) – REQUIRED
- Submit Self-Query NPDB pulled within 30 days from submission at time of presentation- REQUIRED
- BLS
- Board Certified
- Board Eligible
