Requirements:
- 2+ years of experience
- Active RN license in Colorado or compact state
- BLS certification (AHA)
- Hospice and palliative care experience
Duties & Responsibilities:
- Provide patient care in a variety of settings including traditional residential homes, assisted and skilled facilities, hospitals, and the hospice care center throughout Mesa County, Grand Junction, CO.
- Make symptom management & urgent visit needs and attend after-hour deaths for all HopeWest service lines.
- Quickly assess the patient/family situation and possible interventions.
- Communicate thoroughly and specifically to the audience.
- Work from home between patient visits.
