- Experience with perinatal education, diabetes management, and substance use considerations (REQUIRED)
- OH license (or willing to obtain) (REQUIRED)
- 2 years+ of CNM experience (REQUIRED)
- Date of agency's last communication with provider (note in "Additional Submission Details") (REQUIRED)

Benefits:
- $140,000 - $145,000 base salary (includes 200 hours vacation/CME)
- $26.00 per wRVU productivity bonus (no cap)
