- Hospital-employed position with outpatient-only clinical setting
- Small patient panel of 20 patients supports individualized, high-quality care
- Lead a dedicated care team including medical assistant, care facilitator, and care coordinator
- On-site services include digital X-ray, ultrasound, stress tests, Doppler, Echo, EKG, and spirometry
- Associate Center Clinical Director role blends direct patient care with clinical leadership
- Responsibilities include mentoring and coaching primary care providers within the center
- Board-certified geriatric medicine physician required; DEA required
