• Types of Cases: Geriatric medicine, annual Wellness Visits and acute needs
• Credentialing: 90 days

• Board Certified Family Medicine or Internal Medicine
• Certifications: DEA
• Licensure: Active Arizona license or IMLC with LOQ in hand
