This vital role focuses on advanced heart failure, heart transplant evaluations, and LVAD patient management, with no outpatient responsibilities. Specific shift hours are noted as see below.

Responsibilities and Duties

• Advanced heart failure
• Heart transplant evaluations and follow-up
• Post-heart transplant management
• LVAD patients
• Cardiomyopathy
• Pulmonary hypertension
• Cardiac transplant rejection monitoring
• Immunosuppressive medication management
• New patient evaluations and follow-up visits
• Medication management and titration
• Review labs, ECGs, echocardiograms, and biopsy results
• Coordinate multidisciplinary transplant care
• Hospital admissions/discharges (inpatient)
• Patient education
