Expanding inpatient Palliative Care inpatient service. Hospital-employed position. Support 6 patients/day. Monday - Friday (8am-4:30pm/5pm). Focus on symptom management, care coordination, transitions of care, and ensuring treatment decisions align with each patient's goals and values. Assist with discharge planning, including transitions to home-based palliative care or hospice. Shared evening and weekend rotation (only phone consults). Cerner EMR.

Compensation:

Compensation package (annual earning up to $130K)
Guaranteed base salary
Signing bonus (up to $10K)
Relocation assistance (up to $5k/negotiable)

Skills

Requirements:

Board Certified: Family Medicine or Internal Medicine
Acute Care NP preferred
Palliative Medicine/Hospice experience
OH license, or eligible
