Family Medicine - Northern Minnesota
An established healthcare facility is seeking a Board Certified or Board Eligible Family Medicine Physician to lead a collaborative care team. This role focuses on achieving patient care objectives and providing exceptional continuity of care for patients and families within a structured care process model. The incoming physician will join a stable, tight-knit team consisting of four family practice physicians, four advanced practice clinicians (APCs), and dedicated support staff experiencing exceptionally low turnover.
Practice Specifics
Schedule & Call: Features a ZERO Call model designed to optimize work-life balance. All after-hours calls (excluding OB, hospital, and SUD/MOUD call) are managed entirely by a separate, designated call team.
Team Structure: The clinic comprises nine providers total, covering family medicine as well as adult and child behavioral health.
Technology & Support: Equipped with the EPIC electronic medical record system and supported by 24/7 Nurse Triage.
On-Site Diagnostics: State-of-the-art care is supported by PACS radiology, ultrasound, mammography, and a high-complexity in-house laboratory.
