Requirements include an active PT license with the DOH in the state of FL, and providing quality care and coordination of care in conjunction with other healthcare professionals. This position also requires compliance with both federal and state guidelines, best practice standards and effective communication skills. This position is working in the Assisted Living Facility (ALF) setting in Lakeland and surrounding areas in the outpatient and home health settings.
