Primary Care position with one of the largest physician-owned and governed multi-specialty clinics in the Midwest. Dedicated Care Team to assist with care gaps, patient outreach, care management, post-ER and admission monitoring, post-discharge follow-up care, and RX support.Multi-Specialty Group Employee, Outpatient only. No Call. Annual Salary. 401(k) profit sharing retirement plan. Optional health, dental, vision, long-term disability, long-term care and term life insurance, and flexible spending accounts. Up to $150,000 Signing Incentive. Large referral base within clinic of over 290 multi-specialty physicians and providers. Largest Physician-Owned and Governed Multi-Specialty Clinics in the Midwest..
