A well-respected Skilled Nursing Facility in Eureka, CA is seeking a highly motivated Physical Therapist to join its dedicated rehabilitation team. The ideal candidate is passionate about patient care and committed to fostering a friendly, positive, and professional environment while helping residents achieve their highest level of function and independence.
Responsibilities:
Evaluate residents and develop individualized treatment plans.
Provide skilled physical therapy services to improve mobility, strength, balance, and overall quality of life.
Collaborate with interdisciplinary team members to ensure optimal patient outcomes.
Maintain timely and accurate documentation in accordance with facility and regulatory standards.
