Shift Details:

• Schedule Details: Hospital Shifts and possible clinic shifts, 8a-5p with 1 hour lunch
• Number of Shifts Needed per Month: Full time requested (3 week minimum)
• Description: Very busy department needing someone to see follow-ups, post-ops, post-device checks, same day appointments and procedure follow-ups as well as hospital coverage and support for EP
• Certifications Needed: TBD
• EMR: Cerner
• DEA: Required
• Patient Population/Case Load: Adult; Variable in hospital/ 14 patients in clinic

Licensing & Privileging:

• License: AZ license preferred; will look at licensing
• Estimated Privileging Timeline: 30 -60 Days
