What You’ll Do
- Provide nursing care to patients in a home health setting.
- Assess patient conditions, document findings, and execute care plans.
- Collaborate with healthcare providers and family members to coordinate ongoing care.
- Administer medications and treatments as prescribed.
- Educate patients and families on health management and safety.
- Maintain accurate records and ensure compliance with all policies and protocols.
- Respond to patient needs independently while following established guidelines.
- Participate in on-call responsibilities as required.
What You Need
Required:
- Valid Massachusetts LVN/LPN license
- Ability to work independently with critical thinking skills
- Good interpersonal and communication skills
- Reliable transportation and ability to drive in various weather conditions
- Flexibility to adapt to work hours and on-call duties
- Ability to maintain confidentiality and adhere to patient rights policies
Preferred:
- Previous experience in home health or community health care
- Proven dependability in similar roles
