Requirements:

- The qualified candidate will have 2 years of experience performing OASIS assessments and care planning for in-home patients.
Duties & Responsibilities:
- Responsible for the care of patients in their home.
- Perform OASIS assessments and care planning for in-home patients.
- Participate in the call rotation for the department, one in four nights, weekends and holidays.
- Scheduled shifts are days, Monday through Friday.
