What You’ll Do:
- Evaluate and develop treatment plans for patients with physical functioning disorders and wound care needs.
- Administer interventions including manual techniques, ambulation, therapeutic exercises, modalities, supportive devices, and wound care.
- Monitor patient progress and modify treatments accordingly, documenting all actions accurately.
- Provide consultation and counseling to patients, families, and caregivers.
- Communicate regularly with the healthcare team regarding patient status and treatment outcomes.
- Supervise and support PT assistants, rehab aides, or students as needed.
- Ensure compliance with all regulatory and clinical documentation standards.
- Participate in care conferences, reviews, and facility trainings to stay current with best practices.
What You Need:
- Valid state Physical Therapist license
- Relevant experience in long-term care or rehabilitation settings preferred
- Strong communication and teamwork skills
- Ability to work independently and adhere to clinical protocols
