What You’ll Do
- Provide outpatient physical therapy assessments and treatments aligned with patient needs
- Collaborate with healthcare teams to develop effective care plans for inpatient and outpatient settings
- Document patient progress and maintain accurate medical records
- Ensure compliance with all safety and healthcare standards
- Educate patients and families on treatment plans and injury prevention
- Assist in discharge planning and coordinate care transitions
- Contribute to the overall well-being of patients in a critical access hospital setting
What You Need
Required:
- Valid Physical Therapy license for New Hampshire
- Minimum of 1 year outpatient and inpatient acute care experience
- Strong communication and teamwork skills
Preferred:
- Experience working in rural or critical access hospital environments
- Ability to adapt to flexible schedules and multiple patient needs
