PT Outpatient Therapy/Rehabilitation Nurse - Monticello, IN

What You’ll Do:
- Provide outpatient physical therapy services to diverse patient populations
- Develop and implement personalized treatment plans in accordance with clinical standards
- Document patient progress using Cerner charting system
- Collaborate with healthcare team members to optimize patient outcomes
- Assist with scope of practice including occasional float within facility needs
- Cover scheduled shifts during weekdays and occasional Saturday on-call duties
- Support patient recovery in short-term acute care setting
What You Need:
- Indiana State Licensure (or pending)
- BLS certification required
- Minimum 1 year of outpatient therapy experience preferred
- Strong communication and organizational skills
- Ability to work within a team and adapt to facility needs
- Flexibility to work holidays as scheduled
