Requirements:

- Current and active plenary physician license to practice in the state of Pennsylvania - [REQUIRED]
- Board Certification Internal Medicine or Family Practice - [REQUIRED]
- Board Certification in Geriatrics - [HIGHLY PREFFERED]
- Experience or added qualifications in geriatrics - [HIGHLY PREFFERED]
- CDS Certification and DEA Registration required - [REQUIRED]
- Previous supervisory experience - [REQUIRED]
- Minimum of one (1) year of documented experience working with a frail or elderly population - [REQUIRED] * must note to confirm
- Effective skills in physical assessment and chronic disease management for frail older adults - [REQUIRED] * must note to confirm
- Desire to work in a creative alternate health care setting - [REQUIRED] * must note to confirm
- Able to manage changing priorities according to participant need - [REQUIRED] * must note to confirm
- Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED] * no exemption, required at presentation
