Physical Therapist - Home Health - (PT - HH)

Pay Rate: $2508.00 - $2585.00
Travel PT Home Health jobs in Jackson, MI are available for 13-week contracts with day shifts and 40 hours expected weekly. You must have a Michigan license; Home Health experience is preferred, but candidates with less experience may be considered for longer contracts.
Your responsibilities include providing one-on-one patient care in home settings, developing personalized treatment plans, and documenting visits using Homecare Homebase EMR. You will work independently, supporting patients’ mobility and recovery goals.
