- Hospital-employed position focused on proactive and preventive medicine
- See 14 patients per day in a value-based geriatric care model
- Dedicated 65+ patient panel emphasizing keeping patients out of the hospital
- Join a collaborative team of 2 physicians and 2 nurse practitioners
- Board-certified Geriatrician, Family Medicine, or Internal Medicine physician preferred
