What You’ll Do
- Provide compassionate hospice and palliative care to patients in accordance with care plans
- Collaborate with residents, physicians, and interdisciplinary team members
- Assist with patient assessments, comfort management, and end-of-life discussions
- Document patient interactions accurately in medical records
- Educate patients and families about care options and support options
- Ensure compliance with healthcare regulations and facility protocols
What You Need
- Valid medical license in Virginia or eligibility to obtain one
- Prior experience in hospice or palliative medicine preferred
- Strong interpersonal and communication skills
- Ability to collaborate effectively within a team environment
- Commitment to providing empathetic, patient-centered care
