Responsibilities

• Deliver skilled nursing care in patients' homes.

• Coordinate care plans with providers and families.

• Provide education and emotional support.

• Complete accurate clinical documentation.

• Promote comfort and quality of life.

Qualifications

• Active Oklahoma RN license.

• Home Health or Hospice experience preferred.

• Excellent communication and time-management skills.

• Valid driver's license and reliable transportation.
