Physical Therapist (PT) – Remote Therapeutic Monitoring (RTM)

About the Role

Aetos Healthcare is hiring a licensed Physical Therapist to support an App-Based Remote Therapeutic Monitoring (RTM) program focused on chronic musculoskeletal (MSK) conditions.

This role is designed for PTs who want to combine clinical expertise with digital health delivery. You will remotely monitor patient progress, evaluate therapy adherence, conduct interactive monthly assessments, and document care aligned with CPT codes 98975, 98976/98977, 98980, and 98981.

This is a structured, compliance-first program. Strong documentation and workflow discipline are essential.

Patient Population

Chronic low back pain

Cervical radiculopathy

Knee and hip osteoarthritis

Post-procedural rehabilitation

Chronic shoulder and MSK conditions

Functional mobility impairment

Core Responsibilities
Clinical Oversight

Monitor RTM dashboard data and therapy adherence

Review patient-reported outcome measures (Oswestry, PEG, WOMAC, functional scales)

Assess functional improvement and symptom trends

Modify therapeutic recommendations within scope

Escalate red-flag symptoms to supervising providers

Patient Engagement

Conduct structured monthly interactive communication (video or phone)

Provide exercise progression guidance

Address barriers to adherence

Reinforce home exercise programs

Ensure appropriate accumulation of 20+ minutes per month when clinically indicated

Documentation & Compliance

Maintain audit-ready documentation

Track cumulative minutes for 98980/98981

Clearly document interactive communication and clinical reasoning

Follow Medicare and commercial payer RTM guidelines

Adhere to state licensure and supervision requirements

Operational Responsibilities

Stratify high-risk vs stable patients

Support panel growth from 50–150 patients

Track KPIs including:

% patients ?20 minutes

Adherence rates

Functional improvement

Escalation frequency

Qualifications

Required:

Active Physical Therapist license (U.S.)

2+ years of outpatient ortho or MSK experience

Strong knowledge of therapeutic exercise progression

Experience with telehealth or remote patient engagement

Strong documentation discipline

Preferred:

Experience in RTM or RPM programs

Familiarity with minutes-based CPT documentation

Comfort using digital health platforms and dashboards

Experience managing high patient volumes efficiently

Workload Expectations

Flexible, minutes-based schedule

Estimated time for 100 RTM patients:
~33 hours/month (for 98980 only)
Additional time if 98981 add-on minutes are clinically necessary
