Momentum Healthcare Staffing (Momentum HCS) is seeking an experienced Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join an innovative outpatient primary care practice in North Carolina. The ideal candidate will have at least 3 years of experience in traditional primary care and a strong commitment to delivering comprehensive, patient-centered care.
This is an excellent opportunity for a motivated healthcare professional who enjoys working collaboratively with physicians, nurses, and other members of an integrated care team while also having the confidence to practice independently.
Position Overview
The Family Nurse Practitioner or Physician Assistant will provide comprehensive primary care services to a diverse patient population in an outpatient setting. The successful candidate will evaluate and treat patients, develop individualized care plans, provide preventive health guidance, and coordinate ongoing care in collaboration with the clinical team.
Schedule
Monday – Thursday: 7:30 AM – 4:30 PM
Friday: 7:30 AM – 2:00 PM
Full-Time Position
No nights
No weekends
No after-hours call
Key Responsibilities
Perform comprehensive patient screenings, medical histories, and physical examinations.
Evaluate, diagnose, and treat a wide variety of acute and chronic medical conditions.
Analyze laboratory and diagnostic test results and determine appropriate follow-up and treatment.
Develop and implement comprehensive, individualized treatment plans.
Treat common family medicine conditions and order appropriate laboratory and diagnostic testing.
Perform basic office-based procedures within the provider's scope of practice.
Interpret diagnostic and laboratory results as appropriate.
Maintain accurate and up-to-date patient records using the eClinicalWorks (eCW) EHR system.
Communicate test results and relevant medical information through the online patient portal.
Provide ongoing care, counseling, and guidance to patients, including those with complex or critical medical conditions.
Educate patients regarding healthy lifestyle choices, preventive care, and disease management.
Establish strong, trusting relationships with patients and encourage open and effective communication.
Collaborate with physicians, nurses, and other healthcare professionals as part of an integrated care team.
Maintain productivity and quality-of-care standards established by the practice.
Demonstrate professionalism, accountability, and a commitment to exceptional patient care.
Qualifications
Master's degree from an accredited Nurse Practitioner or Physician Assistant program.
Current and unrestricted Family Nurse Practitioner (FNP) or Physician Assistant (PA) license in North Carolina.
National certification appropriate to the provider's discipline.
Minimum of 3 years of active clinical experience in traditional primary care.
Strong clinical assessment and decision-making skills.
Excellent written, verbal, and interpersonal communication skills.
Ability to work effectively as part of a collaborative healthcare team.
Ability to work independently and confidently within the appropriate scope of practice.
Strong commitment to patient-centered and holistic care.
