- Start Date: ASAP

- Schedule: Monday to Friday, 8:00 AM – 4:30 PM
- Setting: 100% Outpatient
- Patient population: Adults aged 55+
- Patient load: 3 to 5 patients per day
- Requirements: Board Certification in Internal Medicine (BC/IM), current BLS certification, prior experience in PACE programs or elderly care is required
