   Duties & Responsibilities:
  

    
• Provide compassionate care, strength and courage to patients and families.
• Help patients identify personal goals of care and help caregivers gain confidence.
• Serve as generalists on all diseases and specialists in symptom management.
• Lead the interdisciplinary team for assigned caseload of patients.
• Provide care in various settings: private homes, nursing homes, group homes, or assisted living facilities.
• Work weekday schedule Mon-Fri 8am-5pm with occasional back-up for weekends and on-call.
