What You’ll Do
- Provide specialized physical therapy services in home health settings to diverse patient populations
- Assess patient needs, develop individualized care plans, and document progress
- Administer therapeutic exercises and modalities as prescribed
- Educate patients and families to promote recovery and independence
- Collaborate with healthcare teams to coordinate care and ensure quality standards
- Support the department as an educator and resource for clinical questions
- Travel throughout the coverage radius to serve patients in various locations
What You Need
Required:
- Valid Louisiana Physical Therapist Licensure
- BLS certification
- 1+ year of recent acute care experience
- Strong clinical skills in medical/surgical care
- Valid driver’s license
Preferred:
- Experience in ER or ICU settings
- Experience with EMR systems, specifically Meditech Home Care
- Ability to travel within a 50-mile radius
- Flexibility to work weekends as rotations require
- Strong communication and patient education skills
