- American Board of Psychiatry and Neurology (ABPN) or American Osteopathic Board of Neurology and Psychiatry (AOBNP) - REQUIRED
- Earliest available start date - REQUIRED
- Number of days per week or weeks per month the provider can consistently commit - REQUIRED
- Willingness to rotate among the three outpatient locations - REQUIRED
- Current New Jersey license (or willing to obtain) - REQUIRED
- DEA and CDS - REQUIRED (for credentialing)
- Experience with Cerner or a comparable EMR - PREFERRED
- Any requested schedule restrictions - PREFERRED
- Any malpractice history, credentialing concerns, or restrictions on privileges - REQUIRED (at presentation)
- NPDB (within 30 days) - REQUIRED AT OFFER
- Whether the provider performs EEG interpretation, EMG, or Botox - REQUIRED
- American Board of Psychiatry and Neurology (ABPN) or American Osteopathic Board of Neurology and Psychiatry (AOBNP)
- Board Certified
