Requirements:
- Master's Degree in Nursing (note in "Additional Submission Details" field) - [REQUIRED]
- Board Certified NNP - [REQUIRED] (cannot accept FNP, PNP, or any other NP certifications)
- AZ License (or willing to obtain) - [REQUIRED]
- Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED] * no exemption, required at presentation
- 2+ years of experience as NNP (note number of years in "Experience" field) - [HIGHLY PREFERRED]
- DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - [REQUIRED] * no exemptions
- NNP - Board Certified (cannot accept FNP, PNP, or any other NP certifications)
- Master's Degree in Nursing
- Board Certified
