What You’ll Do:
- Provide skilled nursing care and oversee Resident care in accordance with care plans
- Administer medications and treatments, including monitoring vital signs and glucose
- Document resident responses and adjustments to care
- Collaborate with RNs, CNAs, physicians, and family members to optimize resident well-being
- Assist with resident transfers, labs, and post-mortem procedures as needed
- Supervise nursing assistants to ensure compliance with care standards
- Maintain a safe, clean, and healthful environment
- Participate in team meetings, care plan updates, and staff training
What You Need:
- Valid LPN/LVN license in Pennsylvania
- Strong communication and teamwork skills
- Passion for resident-centered care
- Ability to pass physical and background checks
- Prior experience in long-term care preferred but not required
